In support of U.S. Department of Health and Human Services’ Healthy People 2020 initiative objectives to reduce health disparities and improve health outcomes for lesbian, gay, bisexual, and transgender (LGBT) patients and families, a new report from the HRSA-supported National LGBT Health Education Center outlines how the patient-centered medical home model can be used to help meet Healthy People 2020 goals and provide culturally appropriate care to LGBT patients and other diverse populations.
