Integrating HIV Screening into Routine Primary Care: A Health Center Model

This thirty-page document provides a model to move a health center to a process by which every patient 13 to 64 years of age is screened for HIV as a routine part of medical care. This model, developed with and piloted by a group of health centers, is a step-by-step approach comprised of 8 steps designed to be carried out over a period of 90 days. The model includes worksheets to track the activities to be completed for each step and references a variety of tools and forms that are listed below. These forms are ready to use or modify.
