Description
This course seeks to provide clarity to new members of the HIV healthcare team on the importance of different parts of the initial visit as well as provide a model for other care teams that they may adopt for their patients with complex chronic and infectious conditions. This program was designed to improve team dynamic, communication, and understanding of roles.
Objectives
- Discuss and provide understanding for each team member's role on the HIV care team
- Review considerations when prescribing an initial medication regimen such as polypharmacy and
adherence barriers. - Provide considerations for analyzing patients' social services and case management needs
- Review options for payor source navigation. Including insurance navigation and federal programs.
- Discuss best practices on providing patient-centered care for med reconciliation, adherence
counseling, and disease state education.
