The Collaboration in Care Conference 2026 brings together HIV and Aging service providers, healthcare professionals, interdisciplinary care teams, community leaders, health system administrators, and policymakers to strengthen partnerships and advance coordinated care for people aging with HIV. Centered on care, connection, and collaboration, the conference will highlight innovative strategies that improve health outcomes, reduce isolation, and expand access to supportive services. Sessions will explore the unique health, behavioral, and social needs of older adults with HIV, while showcasing practical approaches for healthcare and community-based teams to identify needs, coordinate care, and connect individuals to essential resources.
Objectives
- Describe the unique health, behavioral, and social needs of people aging with HIV and their impact on care and quality of life
- Identify evidence-informed strategies to improve care coordination, service access, and health outcomes for older adults with HIV
- Apply person-centered and interdisciplinary approaches that strengthen collaboration between healthcare, HIV, Aging, and community-based service providers
- Build partnerships and referral pathways that promote integrated and sustainable systems of care for older adults with HIV
