Description
The South Carolina HIV PrEP Telehealth initiative was developed after meetings with the State Health Department-sanctioned PrEP Workgroup. The workgroup consists of stakeholders from around the state, and it was noted that there was a gap in provider and patient PrEP knowledge, a low number of PrEP prescribers, and a low number of PrEP prescriptions.
Objectives
Identify at least three patient-, provider-, and system-level barriers that impact retention in HIV prevention and treatment.- Describe evidence-based strategies to integrate HIV prevention and treatment services into routine primary care, including screening and risk assessment across diverse populations.
- Apply at least two whole-person, patient-centered approaches (e.g., trauma-informed care, culturally responsive communication) to improve engagement and retention in care.
- Implement at least one practice-level change to enhance continuity of care for individuals at risk for or living with HIV.
