Objectives

  • Describe common barriers and risks associated with the youth-to-adult care transition for youth living with HIV.  
  • Identify evidence-informed models that support successful transitions.  
  • Explain the roles of pediatric, adolescent, family medicine, and adult providers in coordinated transition planning.  
  • Develop transition planning strategies that address medical, psychosocial, and disclosure considerations.  
  • Apply case-based strategies to prevent loss to follow-up during transition.
