HRSA-defined Population Health Measures Data Collection Tool

This resource was developed by Providence Community Health Centers (PCHC) as part of a New England AETC practice transformation project. HRSA population health measures were added to the PCHC electronic health record to provide a more holistic view of patients' health by ensuring that all the important aspects that impact patient health such as sexual history and living situation, in addition to ethnicity, gender identity, and sexual orientation, are accounted for. The document provides an example of how the information is outlined and organized and can be adapted for your clinic's use. Available in English, Spanish, and Portuguese.
